Picture below are of the S4001 Dual adjustment door hinge. They no longer manufacture it. The Replacement is S4003 and S4004 hinge. Please watch video and use the drawings on the product page links to verify with your hinge before you place your order.

S4003 with the vertical adjustment Set Hinge and S4004 for the Horizontal adjustment Guide Hinge. Use both s4003 and s4004 - where top and bottom hinge get S4004. And the middle hinge get S4003.
