Customer: The home was built in 1999.  Attached are some pictures but we are unable to find any other markings on the door or hardware.  It is the Hoppe that you offer.  Pat. Nr. 5,701,636 on hinge (picture of hinge below)

The hinge in the picture you sent looks to be part number S4004 (you will need to confirm by matching the measurements  in our factory drawing to your hinge)  that gold color is no longer available but other finishes are: https://www.windowpartscenter.com/products/s4004-adjustable-guide-hinge?_pos=1&_sid=e06237071&_ss=r
